Bellus Touch Medspa – GLP Program Terms & Agreement

By purchasing and participating in the GLP (Semaglutide/Tirzepatide) program, you agree to the following terms and conditions:

Medical Evaluation & Approval
All clients must complete a medical evaluation with a licensed healthcare provider before receiving any medication. Approval is based on medical history, eligibility, and provider discretion.

Bellus Touch Medspa does not guarantee approval. If you are not approved, a $50 medical evaluation fee will be retained, and the remaining balance will be refunded within 3–5 business days.

Eligibility Requirements
You must be 18 years or older.
A valid government-issued ID is required.
You must provide accurate and complete medical information.
Providing false or incomplete information may result in denial without refund (except for the evaluation fee).

Medication & Treatment
GLP medications (such as semaglutide or tirzepatide) are prescribed by a licensed provider and shipped to the address provided.

Treatment may include medication supply, provider evaluation, and optional supportive medications such as B12 or anti-nausea medication if applicable.

Results vary and are not guaranteed.

Payment & Refund Policy
Full payment is required prior to processing your order.
Once approved and medication is processed or shipped, all sales are final.
No refunds will be issued after medication has been shipped or dispensed.
If not approved, a refund will be issued minus the $50 evaluation fee.

Processing & Shipping
Orders are processed after medical approval.
Shipping timelines may vary.
Bellus Touch Medspa is not responsible for delays caused by shipping carriers or incorrect address information provided by the client.

Client Responsibility
By participating, you agree to follow all provider instructions carefully, use medication only as prescribed, maintain a healthy diet and lifestyle, avoid alcohol consumption during treatment (recommended), and report any side effects to your provider immediately.

Risks & Side Effects
You acknowledge that GLP medications may have side effects, including but not limited to nausea, vomiting, diarrhea or constipation, and headaches.

Serious risks will be discussed during your medical evaluation.

No Guarantee of Results
Weight loss and results vary by individual. Bellus Touch Medspa does not guarantee specific outcomes.

Communication & Follow-Up
Clients are responsible for completing required forms, responding to provider communications, and scheduling follow-ups if needed.

Failure to respond may delay treatment.

Cancellation & Non-Response
If you fail to complete required steps such as forms, ID submission, or consultation, your order may be delayed or canceled. Refunds will follow the policy stated above.

Liability Waiver
By purchasing this program, you acknowledge and accept all risks associated with treatment and release Bellus Touch Medspa and affiliated providers from liability, except in cases of gross negligence.

Consent
By completing your purchase, you confirm that you have read and understood these terms, agree to comply with all policies, and consent to medical evaluation and treatment if approved.
